Agape Forms

One of the fastest ways to get going in your transition is to get a jump start on reporting and case management with forms.  These are the forms Agape uses for social work, child care, and rehabilitation programs.

Social Work Forms

These forms are used by social workers for case management. Forms for intake, assessments, and ongoing followup visits are the foundation of good case management.

These forms include all the forms used in our rehabilitation program. This includes spiritual, academic, and psychosocial assessments and treatment plans.

These are the medical intake forms for children used by the childcare department. There are more forms coming in the future as additional forms for childcare are created.

Agape Spiritual Curriculum

The following spiritual curriculum is used in Agape’s rehabilitation program.  All children are taken through evangelism and discipleship classes based on their individual needs.  The spiritual foundation is the most powerful and effective teaching provided to children for healing and transformation.

Reasons to Believe Class

This evangelism curriculum introduces children to the gospel of Jesus Christ by looking at scripture from creation to ascension through bible stories and activities.

Grow in Christ Class

This curriculum is for children who have made a profession of faith in Jesus Christ and need discipleship in the basics of the Christian Faith.

This curriculum is for children who have made a profession of faith in Jesus Christ and need discipleship in the basics of the Christian Faith.

Agape Counseling Curriculum

The following counseling curriculum is used in Agape’s rehabilitation program.  All children are assessed and then placed in these classes based on their individual needs.  The curriculum is designed for children suffering from trauma and gives them the skills to cope with overwhelming emotions and build better personal relationships in their life.

Group Counseling

These group counseling sessions take children through understanding and managing their feelings. It provides support for dealing with reintegration and dealing with the relationship issues they may face at home.

Therapeutic Storytelling

This group session takes children through the story of a child that makes lots of mistakes in their life. It provides children with a safe place to see their own life through the eyes of a lovable character and teaches them to make better decisions in their own life.

Nurture Groups

This class teaches children to identify overwhelming emotions and coping mechanisms for dealing with them. These skills are taught in a classroom environment but reinforced on the playground, the classroom, and throughout daily life during the rehabilitation program.

Anger Gremlin Counseling

This counseling curriculum helps children struggling with anger issues. They are taught to recognize and respond to their own anger in a healthy way.

Hurts Habits and Hangups

This addictions curriculum helps children suffering from addictions. It's based on the Celebrate Recovery model of addiction and works well with children struggling with drug and alcohol abuse as well as other destructive behavior.

Family Issues

This class prepares children for being reintegrated back with their families. It discusses issues they will face when back with their families and teaches them healthy ways of dealing with them. This group counseling class also helps address the fears and concerns children may have when returning home.

General Rehabilitation Curriculum

The general rehabilitation curriculum is used in Agape’s rehabilitation program.  These classes help children express themselves and develop skills that prepare them to return to their family and community.

Life Hurts God Heals

This course teaches children to understand how hurts in life impact them. It also teaches them to understand what abuse is and why abuse happens along with ways to deal with difficult situations in a healthy way and understanding how God can heal those hurts.

Planned Activities

This set of planned activities help to foster healthy relationships through team building. Outdoor games and non-classroom activities help get children outside playing together in a safe and healthy way.

Lifeskills

This group of life skills lessons teaches children how to relate to people and deal with daily life in a healthy way. It teaches about friendship, decision making, and other skills for dealing with daily life.

Creative Expressions

These creative lessons help children express themselves in a creative way. Through drama, art, music, and other creative expressions children learn they have more than just a voice, but a creative force within them.

Character Studies

Through these biblical character studies, children learn about important character traits by looking at role models in the bible. They learn about responsibility, honor, kindness, and other important character traits from famous people in the bible.
